Here we are, the start of another exciting year of football & cheer. In order for your son/daughter to be able to participate in all of the games and events we have in front of us, you need to ensure that all required registration and participation paperwork is filled out correctly. Here is a rundown of everything you need to get turned in ASAP:
Birth Certificate with a raised seal. This cannot be a photocopy. You can get raised seal copies at Town Hall for a small fee.
Physical report signed on or after Jan 1, 2017. If your child's most recent physical was in 2016, you need the doctor to fill out the Medical Clearance form. There should be no charge for this.
Signed Code of Conduct
Signed Concussion Acknowledgement
Signed Image Release
Signed Waiver and Release
Correctly Filled out Emergency Medical Treatment form
Signed Participation & ID Card
Copy of most recent school issued report card
Also, if the form calls for the player's signature, you need to have him/her sign, no matter their age. They can practice printing their name.
